Thank you for supporting Providence Community Health Foundation. To purchase tickets to our annual gala dinner and auction night on December 3, 2025, please use the registration form below.

 

PLEASE NOTE:
You have the option below to vault a credit card to use during the auction. Your credit card will not be charged unless you make a donation or purchase something at the event or in our online auction. It is securely held by a vetted third-party vendor. You will have an opportunity to make a tax-deductible donation at any time during the silent auction timeframe. If you would like to make a donation prior to our event, please type a donation amount below.
